Health Insurance card and information.
A list of all current medications you are taking, including vitamins and supplements
A list of your questions and concerns
A copy of any recent chest x-rays, CT scans (not the reports - obtain the actual films or CD)
Copies of records from doctors related to your pulmonary problem
Completed forms provided by this office (see below)
